Melissa Fortenberry
OTR
Occupational Therapist / Certified Vestibular Therapist /
Neurological Therapist
Melissa graduated from Texas Woman’s University, Denton Texas, in 1994, and since then has practiced post-op orthopedics, inpatient rehabilitation, and skilled nursing, focusing on long term acute, and traumatic brain injury.
In 2004, she began focusing on the specialty practice area of Vestibular Therapy (VRT), an exercise-based therapy program that uses repetitive movements to treat the common symptoms of vestibular (inner ear) injury include vertigo, dizziness, and impaired balance, receiving a Vestibular Competency Certificate in 2007 and the LSVT certification in 2015.
She has more than 19 years of experience in the treatment of vestibular, balance and neurological symptoms. As an Occupational Therapist, she helps clients manage or recover from a wide array of complex inner ear diagnosis, including BPPV and labyrinthitis, and disabling conditions such as concussion, ataxia, CVA, and Parkinson’s disease. In addition, Melissa has pursued advanced education to address clients with neurological vision impairments such as convergence weakness and diplopia. She brings to the Affiliates team an OT’s perspective to the many complications that cause adults to suffer from dizziness, to lose their independence or become vulnerable to falling.
Melissa lives in Houston with her husband, where she raised two beautiful daughters and cares for three energetic dogs.
Jennifer Fuentes
PT, DPT
Physical Therapist / Certified Clinical Vestibular Therapist /

Jennifer graduated summa cum laude from Wayland Baptist University with a BS in Biology in 2005 and went on to complete her Doctorate of Physical Therapy at Hardin Simmons University in 2008. She began her career volunteering as a PT with active duty soldiers and airmen in Germany and carries in her heart extreme gratitude for all who have served this country. Jennifer moved to the Houston area in 2016 making it her permanent home after traveling the world as a PT and military spouse.
She has experience working in a variety of therapy settings and treating patients of all ages. Her clinical experience includes treating patients with acute medical illness and deconditioning, stroke, brain injury, other various neurological diagnoses, and numerous orthopedic dysfunctions. Her true passion lies in treating patients for vestibular dysfunction and balance/fall risk. Jennifer received her Certification in Clinical Vestibular Rehabilitation in 2017. She is also a LSVT BIG Certified Clinician since 2019. She values the one-on-one treatment and individualized care she is able to provide her patients at Affiliates Prime as it leads to building trusting patient/therapist relationships. Her favorite part of being a PT is working with her patients to reach their personal goals, allowing them to return to a life they love.
Jennifer has two children and three pups. They live in Pearland, Texas and are thankful to have planted roots there. In her time away from being a PT she enjoys being a mom, watching Astros baseball, lifting weights, dancing, and soaking in the Texas sunshine outdoors with her family.

Kristah Woolley

Physical Therapist
Kristah graduated from Texas Woman’s University with her Doctorate in Physical Therapy in 2013. For her final PT school rotation, she enjoyed working with the team at Affiliates in Physical Therapy so much that she accepted a staff therapist position and has worked with the group ever since.
Kristah recently joined the Affiliates Prime team in 2022. She earned her Orthopedic Clinical Specialist (OCS) with the American Physical Therapy Association in 2018. She is also certified in Fascial Movement Taping through RockTape.
Prior to physical therapy school, Kristah earned her BS in Biology at the University of Texas in Austin and worked for the UT Health Science Center in Houston for 3 years as an organ transplant technologist where she met her husband Nick.
They have been married 8 years and are enjoying being new parents to a young toddler.
Quin Ben
PTA
Physical Therapist Assistant
Quin is a Licensed Physical Therapist Assistant. She graduated from Coleman Health Science with an Associate Degree in Health Science in 2006. Quin gained her experience working in a number of different out-patient clinics, acute hospitals, and rehab hospitals in Texas over the years. She has provided care for patients with numerous neurological and orthopedic injuries of all ages. She joined Affiliates in Physical Therapy in June 2021 and officially joined the Affiliates Prime team in May 2022. Quin is devoted to working as a team player, driven to deliver quality care and exceptional patient outcomes. Outside of work, she enjoys spending time with her family and participating in outdoor activities.
